Pre-Placement Support
Adult Living Solutions’ (ALS) Pre-Placement Support program is a support for our senior community members and their families who feel they are needing education and guidance for an aging family member who lives in their home, or who plan to return home after a hospital or rehabilitation stay. Our professional staff includes Registered Nurses (RN), who will provide a FREE consultation to assist you in creating a plan of care.
Our new program includes:

In-home assessment to determine care needs

Professional guidance on when to transition to a care community

Support & counseling for community resources

Recommendations for a skilled plan of care.

Support for families at a distance who are unsure of what to do to keep their loved one safe until a move can happen.
