Why is your service free to the person seeking to move to a community?


​We work with most of the communities in our area and are given a referral fee when you choose one that we recommend. This is how we can offer help at no charge to our clients.

Do you work with all communities?


We have contracts with nearly all of the communities in the area. Our goal is to match your needs to the right community. We won't recommend a community we have not seen in person or that we don't feel is a good match for you. If, however, none of the communities we would recommend will work for you, we are able to provide you with information for other communities in our area.

How do you determine the best community?


We do a personal assessment to determine preferences, health conditions, care needs, and financial resources. We work closely with nearly all of the communities in Lane County and can help narrow your choices to three or four of the most appropriate communities that best match your needs and desires.

What cities do you cover?


We cover Lane County (including Eugene & Springfield) in Oregon & have contacts through out Oregon & Washington. We can refer to local colleagues in those areas. ​

Under what conditions do you work with individuals on Medicaid?


If Medicaid is the only benefit available for an individual or couple, give us a call! We can give direction of how that system works. If more support is needed, we offer a discounted fee to provide assistance to find a Community that accepts Medicaid.

 

Medicaid provides case workers that do some of what we do. We will work with individuals who will utilize their own resources or long-term care insurance at first and then qualify for Medicaid later, should they qualify. If the first few months of your occupancy are privately paid, we can work with you!​

Why do you offer this service?


We've recognized that there is tremendous need for help navigating the choices in finding a community. Most people we speak with find the process overwhelming and confusing and we want to help. Learn more about us.

What is the referral process?


We begin by talking with the family, spouse or the person in need of care to find out what needs and concerns they might have. Next, we do an in-depth interview to determine health, care needs and resources, personal preferences regarding a studio, one-bedroom, or two-bedroom unit, interests, pets, hobbies, location, etc. We appreciate meeting personally to talk with family and all support people to define needs. Read More.
 

Do you take a person to visit the community?


We will arrange or accompany our clients on tours of the communities we recommend. We help with making sure all concerns are addressed by the community when touring.
 

Who makes the final decision?


Our job is to narrow down the choices but you decide whether one of our recommendations is a good fit for you or your loved one.

What happens if I'm not happy at the community you helped me find?


Thankfully, this rarely happens. We do everything we can to ensure this doesn't happen, but we visit you in your new community. If concerns cannot be addressed to your satisfaction, we will help you find a new home. However, we do very carefully match your needs to help ensure that you make the right decision for the best community.
 

Can I look on my own while using your service?


Once we begin working on your behalf, we ask that you please not contact the communities directly or work with another referral company without talking to us first. This is the only way we can keep our services free to our clients. If you've contacted communities or other referral services prior to using our services, please let us know.

Why should I choose you?


Not only do we have experience after more than a decade of working with communities and residents in our area, but our focus is on you, our client. We follow a strict code of ethics in the referral and placement of every individual. In addition, we're able to draw upon the resources of a local partner, Patient Care Advisors.
